  • File the following forms with the state of Arizona: 1099-R.
  • Filing due dates: File the state copy of form 1099 with the Arizona taxation agency by February 28, 2021 (January 31 in some cases).
  • Reporting threshold: If you file 250 or more 1099 forms with Arizona you must file electronically.
  • When filing federal copies of forms 1099 with the IRS from the state of Arizona the mailing address is: Department of the Treasury , Internal Revenue Service Center Austin, TX 73302.
  • When filing state copies of forms 1099 with Arizona department of revenue, the agency contact information is: Arizona Department of Revenue, 1600 W. Monroe, Phoenix, Arizona 85007-2650.
  • Compliance rules: You may file the Arizona state 1099 copies on paper or on optical media. The ADOR encourages, but does not require, the employer to use optical media filing for 250 or more forms. Arizona accepts CD ROMs and DVDs only. The employer must file the state copies of forms 1099 as part of the reconciliation required in Arizona Forms A1APR (Arizona Annual Payment Withholding Tax Return) and A1R (Arizona Withholding Reconciliation Tax Return).
