  • File the following forms with the state of Indiana: 1099-R.
  • Filing due dates: File the state copy of form 1099 with the Indiana taxation agency by February 28, 2021 (January 31 in some cases).
  • Reporting threshold: If you file 25 or more 1099 forms with Indiana you must file electronically.
  • When filing federal copies of forms 1099 with the IRS from the state of Indiana the mailing address is: Department of the Treasury , Internal Revenue Service Center , Kansas City, MO 64999.
  • When filing state copies of forms 1099 with Indiana department of revenue, the agency contact information is: Withholding Tax Section, Indiana Department Of Revenue, P.O. Box 6108, Indianapolis, IN 46206.
  • Compliance rules: 1099 forms that do not report withholding of Indiana state or local income tax should not be submitted to the department. These returns must be maintained by the taxpayer for three years and made available to the department upon request. WH1 coupons and WH3 reconciliations must be filed even if no tax is reported. While Indiana is a participating state in the Combined Federal and State Filing Program, a copy of the 1099 returns must still be submitted directly to the state of Indiana.
